In college access and student success programs, we spend significant time meeting with students—discussing goals, addressing barriers, celebrating progress, and helping them navigate complex systems like admissions, financial aid, and graduate school preparation.

However, many professionals in these roles were never formally trained to document student interactions. Staff often come from education, counseling, community work, or research backgrounds where case notes were not a standard practice.

Yet for TRIO and similar programs, documentation is essential.

Case notes serve several critical purposes:

• They ensure continuity of support across staff members
